New Patients

Welcome to our practice. So that we may provide the best possible care, we need information about your general medical history as well as your specific ear, nose or throat problem.

Please bring the following 3 things to your first appointment:

1. Photo ID
2. Medical History Form  (Download Here)
3. If the appointment is for your child and a guardian is not present we require a  Written Parental Consent Letter 

It is important that we have copies of any reports or records from other physicians regarding your problem. This would include MRI or CT scans (actual films, not just the report), hearing or balance tests, blood work, sleep studies, and any other information that might be helpful. We require all films to be hand carried to your appointment. If they are not available we regret that your appointment will be rescheduled. If your problem is ear-related, such as hearing loss, balance disturbance or tinnitus, we will usually perform a hearing test at the time of your visit.

We strongly encourage parents to accompany their children to their appointments. If for any reason you are unable to attend these visits, all children under the age of 18 MUST be accompanied by an adult, with written parental consent.

Most insurance company contracts now require that we have prior authorization or a referral at the time of your appointment. If your insurance requires and authorization it is your responsibility to be sure it is in place prior to you appointment. If it is not available, we regret that we will have to reschedule the visit.

If your insurance company requires a co-pay, payment must be made at the time of your visit. We accept cash, checks, and credit cards.

If you cannot keep this appointment, please give our office at least 24 hours notice.
Please be advised that NO SHOW and SAME DAY CANCELLATIONS for NEW PATIENTS will incur a $50.00 fee payable by the patient and NOT REIMBURSED by your insurance.
